Pair Team is one of California’s largest Enhanced Care Management providers.

We’re a medical group built for Medi-Cal. Your care manager is backed by a full team — a nurse practitioner, a registered nurse, and behavioral health support — and they work with your health plan, your doctors, and local community programs so things actually get done.

The program is called Enhanced Care Management (ECM). It’s part of CalAIM, California’s effort to make Medi-Cal work better for the people carrying the most.

Is this really free?

Yes. Enhanced Care Management is a Medi-Cal benefit. There are no copays, no bills, and nothing to buy — ever. Medi-Cal pays Pair Team to provide this program. That’s why there is never a bill for you.

Will this change my Medi-Cal or my other benefits?

No. You keep your Medi-Cal, your doctors, and everything you get today. This adds help — it doesn’t take anything away. And you can stop anytime, no questions asked.

What if I don’t have a stable place to live?

This program was built for exactly that. You don’t need a home address to sign up, and we can meet you wherever you are — a shelter, a clinic, a friend’s place, or on the street.

Can you actually get me housing?

We can’t promise a home — no one honestly can. What we can promise: a real person who helps you apply for housing programs, pulls documents together, follows up on waitlists, and doesn’t disappear after one call.

Who will call me?

A member of the Pair Team care team. The first call usually comes within 24 hours of your form. If you miss it, we’ll keep trying. And if your phone number changes a lot, use the best one you have — a friend’s, a family member’s, or a case worker’s number works too, as long as they can get a message to you.

Can I fill this out for someone else?

Yes. Family members, friends, and case workers send us forms all the time. Put their information in the form, plus the best phone number to reach them — or you.

Is my information private?

Yes. Your information is protected by health privacy laws (HIPAA). We use it to reach you and set up your care — that’s it.
